Managed Care

Fox Valley Ophthalmology is a provider for many managed care contracts. As such, we are happy to file insurance assignments on behalf of our patients participating in these programs, however any co-payment is due at time of service. If you are a managed care participant, please provide us with all pertinent information, referral forms and your participation card. We participate in the following managed care programs.

• Fox Valley Medicine
• Sherman Choice IPA
• Streamwood Medical IPA
• Health Select (formerly Tri-County IPA)

Affordable Payment Plans

Although fees are due at the time of your visit, we will do everything possible to help you structure an affordable payment plan for surgical procedures or long-term treatment. Please discuss any special financial arrangements with our billing coordinator at the time of, or prior to your appointment. For your convenience, we accept cash, Visa, Discover, Master Card and personal checks.

For non-covered services, payment is due at time of treatment. We do however offer assistance with financing for non-covered services including Laser Vision Correction.
